Post Arrest Checklist

Airway

  • Check tube position
  • Reconfirm bilateral A/E
  • Adjust tube tie (around head, not the neck)
  • Consider bite block
  • Replace extraglottic device (if necessary)

Breathing

  • Ventilate 5-10cc/kg
    • Based on Lean Body Mass
    • Average adult is 350-450cc
    • 1/4 to 1/3 of the BVM
  • Ventilate 10-12/min (Standard)
    • Ventilate 8-10/min for COPD/asthma
    • Consider 12-14/min for EtCO2 >45
    • Target End-tidal CO2 of 30–40 mm Hg for a PaCO2 of 35–45 mm Hg
  • Target SpO2 of 94% (Standard)
    • Target SpO2 of 88-92% for COPD
    • Titrate O2 down
  • PEEP 5cm (Standard)
    • Consider 10cm for CHF

Circulation

  • Correct Rhythm
    • Wait 5 minutes to cardiovert SVT
    • Cardiovert WCT immediately
    • Consider amiodarone infusion
    • Consider epinephrine infusion if HR <50 & BP <90mmHg
    • Consider pacing if HR <50
  • Correct Pressure
    • Target MAP of ≥ 65mmHg
    • Fluid bolus
    • Consider push dose epinephrine per Vasopressor Guideline
  • Post Arrest 12 Lead
    • Wait-5-10 minutes for reperfusion
    • 12 lead in car prior to transport

Disability

  • Head of the bed up 30º
  • Check/correct glucose
  • Temperature neutral - No warming
