Whenever any paramedic crew (ACP or PCP) has any patient contact (as defined by policy) that crew must create a PCR.
There is an option available to create a new PCR or to create a copy from another crew using a transfer control copy. In most cases, it’s more efficient for both crews to start the PCR by applying a CAD event and/or using the BC Services Card scanning feature. On layered calls, do a PCR transfer only when a significant amount of information has been entered into a PCR by the first crew. Transfer control, which enables sending a copy of the PCR from one crew to another, can be used at the crew’s discretion if second crew arrives later than the first crew, and the first crew has taken a significant history and/or documented procedures.
First Crew: create a PCR from a CAD event; document information and care given.
Second Crew:(ACP crew for example): create a PCR from a CAD event; document information and care given. If there were any procedures given by Crew A, enter them as “Done By” the first crew member. If an ACP assesses the patient but does not provide any specific ACP care, they still must complete a PCR.
* In the event that one crew accompanies another crew to the hospital with the patient, they are permitted to add themselves to the transporting crew’s PCR instead of creating a separate PCR. If that crew member is a higher license level than the first crew, they will be the Most Responsible Paramedic and be accountable for the patient’s care until transferred to the receiving care location.
